Fill in an appeal form

If you have read our Financial Assistance Scheme Booklet (Adobe Acrobat PDF 96Kb, PDF Help) and now wish to complete an appeal form in order to appeal against a FAS decision, please:

Print out the form, fill it in, sign it/get it signed where appropriate and post it to the PPF Ombudsman. Please note that online forms will shortly be available.

If you wish to take part in an appeal and you have read our guidance FAS - Guidance Note: Interested Persons who are not treated as Parties to the Appeal (Adobe Acrobat PDF 50Kb, PDF Help) and believe that you are an interested person  then please file out the form below and post it to the PPF Ombudsman together with evidence in support where available.
